Glioblastomas (GBMs) are primary brain lesions with poor overall survival. One of the leading factors to this high mortality is the inevitable nature of cancer recurrence. In consequence, early detection of cancer return is critical to patient survival and monitored using serial magnetic resonance imaging (MRI) to detect areas of contrast enhancement indicatory of possible cancer return. However, similarly presenting pseudoprogression (psPD) complicates monitoring as such contrast enhancing lesions present identically under MRI. This paradigm complicates patient management clinically as psPD may be conservatively managed with monitoring and/or steroids while true cancer progression (PD) must be aggressively managed with surgery and treatment revision. In turn, histologic confirmation by neuropathology is critical in resolving novel MRI enhancements, but equally challenged in accurately classifying response as practical clinical guidelines do not exist. To examine this challenge, my project seeks to explore the underlying epidemiologic, transcriptomic and histologic features which characterize these responses using machine learning to derive novel understanding of disease mechanism and clinical guidelines.
